I understand that any type of therapy offered by Caro Rodriguez is an energy-based practice intended to support relaxation and well-being. I understand that Caro Rodriguez is not a medical professional and cannot diagnose conditions nor prescribe or perform medical treatment, prescribe substances, nor interfere with the treatment of a licensed medical professional.
I understand that the products or services provided to me by Caro Rodriguez do not take the place of medical care.
I understand that it is recommended that I see a licensed physician or licensed healthcare professional for any physical or psychological ailment I may have.
I understand that the products and services provided by Caro Rodriguez can complement, but cannot replace any medical or psychological care I may need and/or be receiving.
I acknowledge that it is not possible to estimate the number of sessions I will need to resolve the issue(s).
I further acknowledge that it is not possible to guarantee any specific outcomes for any sessions I receive from Caro Rodriguez.
I understand that a healing crisis and a temporary worsening of symptoms (physical, mental, emotional, and spiritual) may occur.
I agree that this intake form and Waiver is in effect for all the sessions I receive and will not expire unless requested by either party.
